Job Description:
Assisted is looking for a per diem Certified Home Health Aide
for Home Health, supporting our Covina branch covering the west end
of our market, Pasadena to Covina, to pick up about 5-10 visits per
week. You must have a Home Health Aide Certification through the
California Department of Public Health. Assisted offers great and

impact you will make: Assist patient with personal hygiene,
elimination, / nutritional support, ambulation and exercise
according to the patient’s needs and as specified on the CHHA
Assignment. Ensure quality and safe delivery of CHHA services per
written plan of care. Communicate with RN, Rehab Case Manager or
Clinical Supervisor on a regular basis regarding changes in the
patient’s condition and/or needs. Who you are: Completion of an
approved training program pursuant to Federal/State law. Current,
active California CHHA certificate with criminal background check.
Current CPR, health clearance and completed skills assessment. Full
