Job Description:
AccentCare is seeking a dedicated RN Case Manager to join our
team of home health professionals. In this role, you will
coordinate and provide nursing care and support to our patients in
their homes. This involves assessing patient needs, planning and
implementing care strategies, and documenting patient progress.
Effectively collaborating with other healthcare professionals and
ensuring compliance with all state and federal regulations are also
key duties. This position requires a skilled nurse with a passion
for patient care and a strong ability to adapt to the dynamic
environment of home health services. Assess patient health needs
and develop nursing care plans. Implement care strategies and
monitor patient progress. Document patient data and treatment plans
in patient records. Educate patients and their families on health
maintenance and disease prevention. Coordinate with physicians,
therapists, and other medical professionals. Ensure compliance with
healthcare regulations and standards. Manage a caseload of patients
and prioritize needs effectively. Strong interpersonal and
communication skills. Proficiency in clinical assessments and care
planning. Ability to function independently while also
collaborating with a team. Knowledge in using medical software and
database systems. Capability to handle emotional and physical
aspects of patient care. Medical Insurance Dental Insurance Vision
Insurance 401k Valid RN license in the state of practice.
Certification in Case Management (CCM) preferred but not
required.
