  Case Management Travel RN
  
  ---Conduct intake assessments with patients and families, ensuring that all parties feel sufficiently supported.
  
  ---Monitor patient's vitals and report adverse reactions in a timely manner.
  
  ---Develop, implement, and adjust individualized patient care plans that account for physical, financial and any other constraints.
  
  ---Suggest and coordinate appropriate follow-up services and ongoing treatments as needed.
  
  ---Serve as patient, family, insurance, and facility advocates and assist with admission and discharge.
  
  ---Flexibility to travel to the assignment location.
  
  ---Availability to work the facility's required schedule.
  
  ---Willingness to complete onboarding items in a timely manner.
  
  ---Ability to quickly adapt to new environments and strive for the highest quality of patient care.
  
  ---Also referred to as: case manager, travel case manager, discharge planner, utilization reviewer.
  
  Job Requirements
  
  ---1+ year of experience as a Case Management nurse
  
  ---Graduate from an accredited nursing school
  
  ---Current state specific RN license in good standing, or if applicable, a compact state RN license in good standing
  
  ---BLS certification required
